Lumenos Plan Summary

 

Option 1: Lumenos Health Savings Account (HSA) Plan is funded by your own contributions which may be tax deductible. You can set up a separate Health Savings Account or HSA which you can use to pay for medical care and prescriptions

If you open a Health Savings Account with your Lumenos HSA plan, you can save on taxes in three ways:

  1. Contributions may be tax deductible (within certain limits) which can reduce your overall taxes.
  2. Money in a HSA account can earn tax-free interest
  3. Eligible medical expenses are paid for with pre taxed dollars

Option 2: Lumenos Health Incentive Account (HIA) Plan is funded entirely through reward credits you can earn for healthy behaviors. It gives you an account called a Health Incentive Account, or HIA which you can use to help pay for medical care and prescriptions, and can lower the amount you have to spend out of your pocket.

 

Both options provide 100 % preventive care coverage at no additional cost to you. (deductible waived)

When you use our network providers, nationally recommended preventive care services are covered by Blue Cross, with no additional cost to you.

Lumenos with maternity coverage has three deductibles; $1,500, $3,000 and $5,000. Your annual out of pocket limit is equal to the deductible. Blue Cross pays 100% of covered medical expenses for in network services after your deductible.

Lumenos without maternity coverage has three deductibles; $1,500, $3,000 and $5,000. Blue Cross pays 70% of covered medical expenses until your deductible plus your 30% coinsurance equals $5,000. The $5,000 deductible does not have any coinsurance. Blue Cross pays 100% of covered medical expenses for in network services after your deductible.

  • Lumenos Health Incentive accounts is funded entirely through reward credits you can earn for healthy behavior.
  • Lumenos Health Savings Accounts is funded by your own contributions, which may be tax deductible. (within certain IRS limits)
  • You maximize your plan benefits by choosing providers and hospitals within the Blue Cross PPO network.  Out of Network benefits are lower.
