Here is how you would use Lumenos Plans

You decide on the plan design that is right for you Choose from Lumenos Health Savings Account or Lumenos Health Incentive Account and Deductibles of $1,500, $3,000 or $5,000

Unique feature of these plans: Blue Cross pays a coinsurance of 70 % of medical expenses after your deductible. Blue Cross pays 100% of covered medical expenses after your deductible plus your 30% coinsurance amount equals $5,000. The $5,000 deductible plan does not have any coinsurance. Blue Cross pays 100% of covered medical expenses after the deductible.

You go to a doctor using the Lumenos Health If it is preventative care you pay nothing using in-network providers Doctor visit can be paid for by your Health Incentive Account or Doctor visit can be paid for with pre-taxed dollars from your Health Savings Account Doctor visit can be paid for out of pocket until the deductible is met

You have a prescription to fill Go to almost any pharmacy Pay from Health Incentive Account or Pay from Health Savings Account or Pay out of pocket until deductible is met

You go to the emergency room Pay from Health Incentive Account or Pay from Health Savings Account or Pay out of pocket until deductible is met

You're admitted into the hospital Choose from over 400 hospitals in California Pay from Health Incentive Account or Pay from Health Savings Account or Pay out of pocket until deductible is met Once the deductible is met, Blue Cross pays 100% of the cost of the room, surgery and all other care

You visit the dentist You can add Dental Insurance to this plan Choose a PPO or HMO model Dental care contributes to your overall health

This Lumenos plan does not cover maternity.

Lumenos Health Incentive accounts is funded entirely through reward credits you can earn for healthy behavior.

Lumenos Health Savings Accounts is funded by your own contributions, which may be tax deductible. (within certain IRS limits)

You maximize your plan benefits by choosing providers and hospitals within the Blue Cross PPO network. Out of Network benefits are lower.
