You go to a doctor

  • Blue Cross pays 100 % after deductible

You have a prescription to fill

  • Go to almost any pharmacy
  • Pay $10 copay for generic drugs after deductible
  • Pay 30% of negotiated fee for self administered injectables, except insulin, after deductible
  • Pay $30 copay after annual deductible

You go to the emergency room.
  • Pay $0 after annual deductible

You're admitted into the hospital

  • Pay $0 after annual deductible
 hospital room

You visit the dentist

  • You can add Dental insurance to this plan
  • Choose a PPO or HMO model
  • Dental care contributes to your overall health
 dentist with patient

 

The HSA Compatible PPO Plan does not include maternity benefits.

You maximize your plan benefits by choosing providers and hospitals within the Blue Cross PPO network.  Out of Network benefits are lower.
